5 Essentials Survey open to parents

The 5 Essentials Survey is now open to students, teachers and parents through March 11.

Administered by a third party, the UChicago Impact at the University of Chicago, the 5 Essentials Survey was implemented by ISBE more than eight years ago as a tool to gain confidential feedback about the climate and culture of a school or a school district from parents, students and teachers.

The survey, which ISBE now provides an opportunity for students and teachers to take annually, asks questions about students’ experiences, attitudes, activities in school, instruction, professional development, the school as a workplace and parents’ overall relationship with the school.

The five indicators that affect and predict school success are: 

  • Effective Leaders
  • Collaborative Teachers
  • Involved Families
  • Supportive Environments
  • Ambitious Instruction 

All teachers and fourth through twelfth-grade students at Mahomet-Seymour are also responding to this survey.  5 Essentials Reports will be generated for each of school.  These reports are sent to schools and districts in April 2022 and are also included on the State School Report Card.  

Parents are also able to take the survey. If a school district has 20-percent parent participation, a parent report is generated for the school.
