Mahomet-Seymour 2020-21 Registration Information
The Mahomet-Seymour School District has opened 2020-21 registration.
Parents were asked last week via email to decide if their child would attend school in-person or online. But parents are still required to visit Skyward to register their student, submit the proper forms and pay the registration fee associated with school and extracurricular activities.
Registration is required to be completed before August 19, the first day of school.
Mahomet-Seymour Junior High and Mahomet-Seymour High School are also hosting TOOLS and GRIP for sixth grade students and freshmen. Students must be registered for school prior to completing these transition programs.
If you have any questions, please contact the office of the school(s) where your child(ren) will be attending in the fall.

K-5: $110
6-8: $135
9-12: $170
MAHOMET-SEYMOUR REGISTRATION REQUIREMENTS FOR THE 19-20 SCHOOL YEAR:
All Pre-K, Kindergarten, 6th and 9th-grade students must submit the state-required physical forms and immunization records as listed below before they will be allowed to enter school in the Mahomet-Seymour District.
State of Illinois requirements by grade are:
Pre-K:
- Birth certificate, must be a certified copy
- Current physical dated within one year of first day of school.
- Immunization records that reflect the following:
- 4 doses Tetanus
- 3 doses Polio
- 1 dose MMR
- 3 doses Hepatitis B
- 4 doses Hib titer
- 1 dose Varicella (chicken pox) vaccine
- Pneumococcal series, or one dose after age 2
Kindergarten:
- Birth certificate, must be a certified copy
- Current physical dated within one year of first day of school
- Immunization records that reflect the following:
- 4 or more doses Tetanus – most recent must be dated after 4 years of age
- 4 or more doses Polio – most recent must be dated after 4 years of age
- 2 doses MMR
- 4 doses Hib titer (not required after 5th birthday)
- 2 doses Varicella vaccine – first on or after 1st birthday, second no less than 28 days later
- Current Dental Examination
- Current Vision Examination completed by an eye doctor
6th grade:
- Current physical dated within one year of first day of school
- Immunizations as per requirements listed above, plus
- Proof of having received aTdap booster, a second varicella vaccine and,
- Proof of having received one Meningococcal vaccine
- Current Dental exam
9th grade:
- Current physical within one year of first day of school (must be a complete physical, NOT a sports physical)
- Immunizations as per requirements listed above, plus
- Proof of having received at least one Tdap booster, a second varicella vaccine and,
- Proof of one Meningococcal vaccine
- Current Dental exam
12th grade
- Proof of having received 2 doses of Meningococcal Vaccine w/ the 2nd after age 16
6th thru 12th grades: Proof of having received at least one Tdap booster
2nd, 3rd, 6th thru 12th: Proof of having received two doses of Varicella (chickenpox) vaccine.
6th, 7th, 8th & 9th grades: Proof of having received one dose of Meningococcal vaccine
All students who are new to the district in any grade will be required to provide a certified copy of their birth certificate, a current physical, complete immunization records, current vision exam, and proof of residency.
